REGISTRATION

STEP 1: APPLICATION

The following forms must be completed for application:

The date the forms & fee are received at St. Timothy is the date the child(ren) will be placed on the wait list.

West entrance door (bell tower) open 7:00 a.m. - 5:30 p.m. Monday – Friday.  

Please call (319) 988-3633 in advance for a tour.

STEP 2: ENROLLMENT

After notification of enrollment the following forms must be completed within ONE WEEK after receiving email confirmation.

         

          1. ACH Form: ProCare Tuition Express - (for electronic payments). Complete, sign and return with a voided check.

          2. Allergies and Medication Information (if needed):
